Use the MEDCHART Lookup Tools

All MEDCHART users have access to the Lookup tools from any of the MEDCHART modules. Users can look up a specific Soldier and their demographic information or the description and organizational chart of a specific unit. This information is stored in the MEDCHART Framework data base which is updated daily from the Integrated Total Army Personnel Database (ITAPDB).


ClosedLookup Service Members

Use this procedure to look up a Soldier by their Social Security Number (SSN) or last name.

  1. From the MEDCHART section of the MEDCHART Home page, click the Lookup SM link.


    The Lookup SM page displays.


  2. In the Soldier Search and Select section, enter at least one of the specified search criteria, and then click the Search button.

    Note: -_The more information entered the more refined the search results will be.

    Filter Description
    First Name Enter at least one character of the Soldier's legal first name.
    Last Name Enter at least one character of the Soldier's legal last name.
    SSN Enter at least four digits of the Soldier's SSN
    EDIPI Enter the Soldier's full ten digit Electronic Data Interchange Personal Identifier (EDIPI) number.

    A list of results display at the bottom of the Soldier Search and Select section that match the search criteria.

  3. To allow for easier viewing of the displayed information, use the following features:

    • If the search returns more than one page of results, click on a page number link (located at either the top or bottom of the page) to move to a different page.

    • To re-sort the displayed information in ascending/descending order, click on any column header link.

  4. To view the Soldier's demographic information that is supplied by the Total Army Personnel Database (TAPDB), click the Select link that corresponds to the Soldier in the list of results.

    A new box opens up below the list of results that contains the Soldier's demographic information.


    Note: -_To hide the Soldier's information, click on the banner bar at the top of the box. To display the information, click on the banner bar again.

  5. To view previously entered contact information for this Soldier, click the Soldier's Contact Information link to view.

    The Soldier Contact Information window displays. The information displaying in this window includes the Soldier Data from TAPDB and can include two additional sets of contact information if they were previously entered in MEDCHART.

    • If the SM does not have an existing record in MEDCHART the contact information is not available and the fields are not editable.

    • If the SM does have an existing record in MEDCHART the contact information entered by a previous MEDCHART user displays and users with appropriate permissions may edit the fields as appropriate.

      Note: -_MEDCHART retains only the last two updates to the Soldier Demographics Contact Information. These changes will not be updated in the Soldier's record in TAPDB.

  6. To send an e-mail to the Soldier, click the hyperlink that displays for the MIL/GOV Email address in the Soldier Data from TAPDB section.

    The user's default e-mail program creates a new e-mail message to the displaying e-mail address.

    WARNING! -_Do not disclose PHI information in an e-mail to a Soldier.

  7. To close the Soldier's Contact Information window, click the OK button.

  8. To return to the MEDCHART Home page, click the Home tab at the top of the page.

ClosedLookup UIC

Use this feature to look up the description and organizational chart of a military unit using its Unit Identification Code.

  1. From the the MEDCHART section of the MEDCHART Home page, click the Lookup UIC link.


    The Lookup UIC page displays.


  2. In the UIC Lookup Utility section, enter a six-character UIC in the box provided, and then click the Search button.

    The UIC utility information and organizational chart displays.

    Note: -_If the organization chart for the searched UIC does not exist the following message displays: Could not find a(n) Org Chart for "WXXXXX" (Searched UIC).

  3. To the right of the Search button, review the UIC Utility information. See the table below for descriptions.

    UIC Information Description
    XXXXXX:Unit Name The six-character Unit Identification Code and the name of the unit.
    Region The State to which the unit is assigned.
    OFTS Parent Command The State to which the unit is assigned.
    DIV(R) The State to which the unit is assigned.
    Ending Date

    Indicates the date a UIC was closed.

    Note: -_If Open displays, it indicates that the UIC is still active.

    Usage Status A status of Populated indicates that service members are assigned to the UIC, while the Not Populated status indicates no service members are assigned.
  4. Below the utility section, review the selected UIC's personnel strength information. See the table below for descriptions.

    Item Description
    Name The unit name/description.
    Rollup Strength The total number of Soldiers assigned to the unit and it's associated child units.
    Unit Strength The number of Soldiers assigned to the unit.
  5. To view the selected UIC's ancestor/parent unit Personnel Strength statistics, hover over the appropriate red UIC box.

    Note: -_The ancestor/parent units are organized above the searched UIC box with the highest ranking unit at the top.

    The UIC box is highlighted in purple and the personnel strength information for that unit displays.

  6. To view the selected UIC's child unit Personnel Strength statistics, hover over the appropriate green UIC box below.

    The UIC box is highlighted in purple and the personnel strength information for that unit displays.

  7. To view the utility information for one of the ancestor or child units, click the corresponding box.

    The UIC utility and organizational chart displays for the selected UIC.
